3. Download the application material now and mail or fax it to us. The Uniform Residential Loan Application Form 1003 is available in two formats:

  • Web format. This can be viewed and printed directly from your web browser. The formatting has been altered to accommodate web viewing.
  • Adobe PDF (Portable Document Format) The Adobe PDF version retains the standard formatting of the 1003 form. Viewing the PDF document will require the use of Adobe Acrobat Reader

Both versions of this document consist of 4 pages. Please note: the Uniform Residential Loan Application Form 1003 is a legal size document. Please print on legal size paper or shrink to letter size.

Documents Required

Please provide:

  1. Contract for Purchase
  2. Contract for Sale (if applicable)
  3. Last two years W-2's
  4. Most recent pay stubs (1 month's worth)
  5. Monthly Bank Statements on all accounts – Checking, Savings, & Brokerage accounts for the past 2 months
  6. If self employed, business and personal tax returns for the past 2 years and year-to-date Profit & Loss Statement
  7. Realtors' names and phone numbers
  8. Homeowners insurance Agent’s name and phone number
  9. Copy of Driver's License

Depending on the type of loan program you are qualifying for you may need additional information. If applicable you will need:

  1. A copy of your divorce decree with all pages including separation agreement
  2. A copy of your complete bankruptcy papers and discharge papers
  3. A copy of the front and back of your Alien Registration Card (Green Card)
